Behavioral Health Integration (BHI) and Collaborative Care Model (CoCM) are an innovative approach to providing mental health care that bring mental health professionals into the primary care setting. This makes it easier for patients to get the mental health care they need without having to go to a separate clinic or office. This innovative approach offers a number of benefits to patients, including improved access to care, coordinated care, earlier identification and treatment, reduced stigma, and increased patient satisfaction.
RECOGNIZING THE VITAL CONNECTION
When you receive BHI/CoCM services, your primary care provider collaborates with a dedicated team of behavioral health professionals. This team includes a behavioral health care manager/therapist, a psychiatric nurse practitioner, and other mental health professionals. Together, they develop and implement a personalized care plan tailored specifically to you.
